Anticipated Compensation: $22/hour
Work Schedule: Monday – Friday during standard business hours
This is a remote position - candidates must reside in the state of Texas.
The Angel Link Outreach Coordinator plays a key role in connecting clients with essential Health Home services by managing enrollment, admissions coordination, and client support. This role requires strong customer service abilities, a client-centered approach, and the ability to balance multiple priorities in a fast-paced environment.
In This Role, You Will
- Perform outreach activities to enroll clients in the Health Home program.
- Assist with Angel Link client inquiries and admissions procedures.
- Manage the admissions process and coordinate services for the Health Home population.
- Maintain accurate and complete records and documentation as required.
- Communicate clearly and effectively with clients, clinicians, and internal staff; build strong, positive relationships.
- Perform additional duties as assigned to support the team and organization.
Qualifications
- 3+ years of customer service experience
- High School Diploma or equivalent
- Excellent verbal and written communication skills
